One Sheet
Sometimes it helps to have a little something prepared to provide others about your childs needs. Be it a school teacher, a baby sitter, a coach, or anything in between.
I came up with this “One Sheet” as a sort of cheat sheet for my child and found it so helpful that I wanted to share it 100% FREE with you in the hopes it helps you as much as it has me.
While this downloadable form can be used on any device, I do recommend you use “Adobe Reader” to complete the form to ensure full functionality. A link to download a free version of Adobe will be included below as well for your convenience.
Download your copy of the One Sheet below in editable PDF format. You can type directly into the document or, if you are a more pen and paper person, print it out and fill it in by hand. There is no “right” or “wrong” way to complete it. This sheet is meant to be a helpful tool for you, so use it however you see fit.
